Liability Waiver & Release of Claims

Assumption of Risk

I understand that participation in outdoor and guided activities, including but not limited to hiking, fishing, hunting, wildlife observation, travel in remote areas, and use of outdoor equipment, involves inherent risks. These risks may include, but are not limited to, slips and falls, uneven terrain, weather conditions, water hazards, wildlife encounters, equipment failure, and physical exertion that could result in injury, illness, or death.

I knowingly and voluntarily assume all risks associated with participating in activities provided by 3 Rivers Guiding Service, whether such risks are known or unknown, foreseeable or unforeseeable.

Release of Liability

In consideration for being allowed to participate, I hereby release, waive, discharge, and hold harmless 3 Rivers Guiding Service, Anthony Rolli, and any associated assistants, employees, or volunteers from any and all claims, demands, actions, or causes of action arising out of or related to any injury, loss, damage, or death that may occur while participating in guided activities, including those caused by negligence, to the fullest extent permitted by New Hampshire law.

Medical Responsibility

I certify that I am physically capable of participating in these activities and have no medical conditions that would prevent safe participation. I understand that I am responsible for my own medical care and expenses in the event of injury or illness. I authorize emergency medical treatment if deemed necessary and agree to be financially responsible for any such treatment.

Emergency Contact

I understand that I am required to provide an emergency contact name and phone number. I authorize 3 Rivers Guiding Service or emergency personnel to contact this person in the event of an emergency.

Personal Responsibility

I agree to follow all instructions and safety guidelines provided by the guide. I understand that failure to do so may increase the risk of injury. I acknowledge that I am responsible for my own actions during the activity.

Photo / Media Release

I have the option to grant permission for photographs or video taken during the activity to be used for promotional or educational purposes by 3 Rivers Guiding Service by checking the designated agreement box.

Minor Participant Consent (If Applicable)

If the participant is under the age of 18, I certify that I am the parent or legal guardian of the minor named above. I consent to the minor’s participation in guided activities provided by 3 Rivers Guiding Service. I agree to all terms of this waiver on behalf of the minor and myself, and I release and hold harmless 3 Rivers Guiding Service and Anthony Rolli, and any associated assistants, employees, or volunteers from any claims arising from the minor’s participation, to the fullest extent permitted by New Hampshire law. Minors 17 and under must be accompanied by legal adult at all times.

Governing Law

This agreement shall be governed by and interpreted according to the laws of the State of New Hampshire.

Acknowledgment

I show that I have read this waiver in its entirety, understand its contents, and agree to its terms voluntarily by submitting this form and checking the designated agreement box.
